I'm getting worse as a player and I can't understand why

Annual physicals are good advice.
As we age, twice a year is even wiser.
Blood work etc etc.
I have a female Doc, and always look forward to the, turn your head to the side and cough, part of the exam. 😱

I had a female primary care doctor for a few years. I learned early on that she didn’t care if I were male or female or anything in between. She was a pretty good MD too, and helped me through a couple of difficult problems with my heart.
 
We become far more self-critical as we develop our skills. I think I would look at switching to a different regime - do different things on different days. Pick a random piece of music that's in a different genre entirely perhaps the Bach cello suites - there's an edition for sax...

I didn't mind which doctor I saw except I'm 'old fashioned' enough to request a gentleman for anything involving the male extremities, especially the 'digital examination'.
Though thinking of it now a lady might have smaller fingers.
 
The first thing that comes to my mind is, are you sure that the issue isn't with your saxophone, MP, or reed? Assuming that you are playing only one.
 
After quite some time I'm back for good. And I'm a lot better.
Is it that my practice is Uber focused?
Was I burned out from work family and am I now better ?
Are those ups and downs normal?
I'm not sure.but I'm back ✊

This is normal, IMO. Everyone hits plateaus now and then. There are good days and bad. Like others have alluded to, take a few week break, choose something else to work on for the time being, just spend a few weeks working on fun stuff, visit friends, do something leasurely with the family or family member, etc.

When your mind has had a rest, then you'll be able to tackle what you were plateau'ed on previously, and can hit it with new vigour. 😉

(Now if I could get Woody Herman's Four Brother's T=200 up to speed ...) 😛

Most of it was probably in your head to begin with. You practice and you start getting into stuff that is more demanding and seems to slow down your progress. But in reality it is not.

If you record yourself in regular intervals or else if you had gone back to some stuff that you thought was difficult a few months earlier you might have noticed that those pieces weren't that difficult anymore despite not practicing them in awhile.

It's a bit like doing a jigsaw puzzle where you have to spend some time sorting pieces before you "see progress"
